Admissions suspended for Madison nursing home

A state investigation has lead to new troubles for a nursing home in Madison.

The Health Department ordered Imperial Gardens Health and Rehabilitation not to admit any more new residents and imposed a one-time state civil monetary penalty of $5,000.

The suspension of admissions to a nursing home can occur when conditions are determined to be detrimental to the health, safety or welfare of its residents.

The suspension is effective until conditions have been and continue to be corrected.

Inspectors found numerous violations at the 165-bed home, including violations of standards within the administration, performance improvement, physician services, nursing services and laboratory services.

Aside from the penalties given by the Health Department, the Centers for Medicare and Medicaid Services has notified the facility that their Medicare provider agreement will terminate Tuesday.

The facility has 30 days to relocate Medicare and Medicaid residents, a time during which a special monitor has been appointed to ensure the health and safety of residents.
