NASHVILLE, Tenn. -

After receiving a citizen's tip, police located three teens who escaped a state facility in Donelson early Wednesday morning, injuring a guard in the process.

Eighteen-year-old Porsha Phillips, and two minors, a 17 and 15 year old girl, were taken into custody at the intersection of Gallatin Road and Woodruff Street, near Kroger and Firestone.

According to police, the trio assaulted a guard and took his keys and car in order to escape the Clover Bottom Development Center located on Stewarts Ferry Pike around 1:15 a.m.

Clover Bottom is a facility for people with intellectual disabilities who are facing criminal charges.

Police recovered the guard's vehicle off of Hermitage Avenue.

The guard suffered non-life threatening injuries.

Phillips remains in custody and is charged with assault, vehicle theft, escape and contributing to the delinquency of minors.

The two other minors remain in custody at the Metro juvenile detention center.
