LEBANON, Tenn. – A Lebanon woman arrested for DUI twice this year is behind bars in Wilson County following an accident Friday afternoon that left a man and his four-year-old grandson dead.
Police determined 44-year-old Barbara Ann Mayfield to be the at-fault driver of the crash that occurred just after Noon at the intersection of East High and Cedar streets in Lebanon.
James A. Barrett, 69, and his grandson, Alton, died in the four-car collision.
One other person was critically injured and taken by helicopter to Vanderbilt University Medical Center in Nashville.
Authorities said at the time of the accident, Mayfield was driving on a revoked driver's license stemming from a DUI conviction in April.
In June, she was arrested on two separate occasions, once for DUI and driving on a revoked license, as well as for violating her probation.
At the time of Friday's accident Mayfield was out of jail on bond.
Mayfield was charged with two counts of vehicular homicide, one count of driving under the influence and driving on a revoked license.
She is currently being held in the Wilson County jail.
